Membership is based on number of employees, currently listed on the employer's payroll. The following is a pricing schedule for up to 50 employees: 

1-3 employees = $365/yr.
4-10 = $465/yr.
11-15 = $515/yr.
16-25 = $570/yr.
26-50 = $775/yr.
